Please read the information provided below on Sports Physicals, Immunizations and Medications. Sports physicals are required for 5th through 8th grade students for any after school athletic activities including intramurals and are due 2 weeks prior to the start of the sport/activity season.
If your child is sick or injured, please refer to the document, When to Keep Your Child Home and notify the health office or main office each day your child will be absent. Also, please keep your contact information current so you may be reached in the event of an emergency.
New Information for the 2024-2025 School Year
Dear Parents and Guardians,
The sports physical packets have changed. Please see your Primary Care Doctor (Pediatrician) for clearance to participate in sports. The new forms are on our website. The Doctor keeps the physical and the only papers returned to the school are the clearance form completed by the doctor and the individual sign off sheets that you have read the pamphlets on sudden cardiac death, concussion and opioids. Any physical turned in last school year that is still current, but is due to expire after the first day of practice for that individual sport, will be good until that sport season is completed. All physicals that are over 90 days old from their original date of completion will need to fill out a health history update, which is completed by the parent. The clearance forms still need to be reviewed by our School Doctor, so please get them in, in a timely manner.
