Please read the information provided below on Universal Health Forms, Immunizations and Medications. Universal Health Forms are required for Pre-K and Kindergarten students. All new transfer students entering from another state or country are required to provide new Universal Health Form and updated immunizations prior to September 1st.

If your child is sick or injured, please refer to the document, When to Keep Your Child Home and notify the health office or main office each day your child will be absent.  Also, please keep your contact information current so you may be reached in the event of an emergency.
